What you provide

The form asks for your name, email address and the service you need. A message, phone number and appointment city or area are optional. Masha uses these details to respond to your request, discuss availability and arrange the next steps.

Please do not include Social Security numbers, tax returns, identity documents or financial account numbers in this form. Ask Masha how to provide any documents needed for your service.

How your request arrives

The form sends the information you enter through FormSubmit to Masha’s business email inbox, which uses Google Workspace. FormSubmit provides a verification step on the next page to help prevent spam. Its service and Google’s verification service may process technical information and use cookies.

If you choose “Email instead,” your selected email service handles sending your message directly to the business inbox.

Questions about your information

You can contact masha@balanceandcowa.com to ask about information you submitted or request a correction or deletion. Masha will respond to discuss your request and any records that need to be retained.

This notice covers website inquiries. The arrangements for documents and records needed for a tax preparation or notary service can be discussed before you provide them.
